I am writing to request any report(s) of investigation(s) into the cause(s) of the chemical spill at Guy's Hospital in August 2025 and the response(s) to the spill held. I also request any report(s) of lessons learned as a consequence of this. The chemical spill to which I refer was reported by the BBC below: https://www.bbc.co.uk/news/articles/c4glpkdxn3go Given the potentially sensitive nature of this information, I ask you to redact any exempt information instead of refusing disclosure. This would be in accordance with guidance on best practice from the Information Commissioner’s Office.
