I write to request information under the Freedom of Information Act 2000 in relation to a serious incident involving a London Fire Brigade firefighter,xxxxxx at or connected to xxxxxxxx Fire Station. This request relates to an incident in which xxxxxx reportedly fell or jumped from height (believed to be from a training tower), resulting in serious injury. Given the significant public interest in firefighter welfare, mental health, and organisational accountability—particularly following previous cases such as Jaden Francois-Esprit—I request full transparency on the following: 1. Incident Details * The date, time, and exact location of the incident involving xxxxxx. * Confirmation of whether the incident occurred during training, operational duty, or otherwise. * The official classification of the incident (e.g. accident, self-harm, attempted suicide, or other). 2. Internal Reporting & Investigation * Copies of any incident reports, internal investigations, or reviews conducted. * Whether the incident was referred to: * The Health and Safety Executive (HSE) * The Metropolitan Police * Any external safeguarding or oversight body * * Outcomes of any such investigations. 3. Welfare & Risk Management * Details of any prior welfare concerns, including: * Mental health referrals * Occupational Health involvement * Any recorded indicators of distress or vulnerability * * Confirmation of whether a risk assessment had been conducted prior to the incident. * What support measures were in place at the time. 4. Post-Incident Actions * What immediate actions were taken by the Brigade following the incident. * Whether any policy, procedural, or operational changes were implemented as a result. * Any internal communications issued to staff regarding the incident. 5. Recording & Transparency * Whether this incident has been formally recorded within LFB systems as: * A serious injury, * A near miss, or * A mental health-related incident. * * If not recorded, please explain why no formal record exists. 6. Whistleblowing & Internal Escalation * Whether any whistleblowing reports or internal complaints were raised in connection with this incident. * Details of how those concerns were handled. 7. Duty of Care & Accountability * What steps the Brigade has taken to ensure lessons learned from this incident have been applied across the organisation. * Identification of any failures in supervision, welfare provision, or management intervention. Important Context I am aware of eyewitness accounts and corroborated information, including details provided by individuals present at or connected to xxxxxx Fire Station, indicating that: * There were multiple opportunities for intervention prior to the incident. * Concerns regarding the individual’s wellbeing may have been known but not acted upon. Additionally, elements of this information were verbally confirmed by a senior LFB officer (Tamer Ozdemir) in front of an independent civilian witness, reinforcing the credibility of these concerns. Given the seriousness of these allegations, I expect full disclosure. Public Interest Justification This request is made in the strong public interest, particularly in light of: * The findings of the Independent Culture Review of the London Fire Brigade, which highlighted systemic failures in culture, leadership, and welfare provision * Previous fatalities and mental health-related incidents within the Brigade * Ongoing concerns regarding the treatment of firefighters and suppression of internal issues Transparency in this matter is essential to ensure accountability and public trust.
